If during the education the student will apply for a driving license, an additional statement is required (please complete) – Statement of the driver’s health, which in the case of a minor student is completed by the parent / guardian (Article 78 of the Act on vehicle drivers) – link
Legal basis: Regulation of the Ministry of Health of August 26, 2019 on medical examinations of candidates for secondary schools or universities and qualifying vocational courses, students and students of these schools, students, participants of qualification vocational courses and doctoral students (Journal of Laws of 2019, item 1651).
